Welcome to The Phoenix Kids Club,

Thank you for choosing the Phoenix Kids Club for Infants and Tots. as your choice of care for your child. Please complete the following steps to complete enrollment:

Step 1:

Contact the facility to ensure space is available for your child(ren)'s age group.

Step 2:

If space is available, ask to schedule a tour.

Step 3:

Upon completion of the tour, visit our website at www.phoenixkidsint.com to pay

the applicable registration fee.

Check your email

(Please check your SPAM folder just in case.)

Step 5:

Download and print the application for enrollment. This document can also be provided by your new facility director. Just come by to pick it up. WATCH THE VIDEO in your email as you are going through the enrollment packet. Fill out your Enrollment packet completely and contact the facility director to schedule your child(ren)s start date.

Step 6:

Bring your enrollment packet and provide a copy of your child(ren)s up to date shot record, and a copy of your child(ren)'s birth certificate.

Step 7:
